For sure a fire helmet, not sure about the skull? It is not pictured in the fire and police books by Radecke

I bought a surplus pith helmet when I was 12 that had a similar skull and cross. Turned out to be from the Bannerman Catalog, and was probably from a lodge or fraternity.
 
Correct Peter ! That is a U.S. skull from a Fraternal order cork or felt helmet. I have the skull and will post a photo later. Corky
